Generic
Nitrofurantoin (Sustained Release)
Description
Ofuran SR is an antibiotic used to treat and prevent uncomplicated urinary tract infections (UTIs) caused by susceptible bacteria. It kills the bacteria in the urinary tract, preventing the infection from spreading. However, it is not effective against viral infections. This sustained-release formulation ensures more extended action, reducing the need for frequent dosing while maintaining effective antibacterial activity.
Benefits
Ofuran SR eliminates bacteria from the urine to provide fast-acting relief from urinary tract infections. Its extended-release formula helps maintain a steady concentration in the body, leading to better bacterial eradication and fewer recurrences. It is commonly prescribed to prevent recurrent UTIs, especially in individuals with frequent infections.
Ingredients
Each Ofuran SR tablet contains Nitrofurantoin SR, an antibiotic that targets Gram-positive and Gram-negative bacteria responsible for urinary tract infections.
Uses and Doses
Ofuran SR is prescribed to treat and prevent uncomplicated urinary tract infections.
- Adults (Acute UTI Treatment):
- 50-100 mg four times daily for 7 days or 3 days after obtaining a sterile urine culture.
- Sustained-release (SR) tablets: 100 mg twice daily for 7 days or 3 days after obtaining a sterile urine culture.
- Adults (UTI Prevention):
- 50-100 mg at bedtime for up to 12 months.
- Children (1 month and older):
- 5-7 mg/kg/day in divided doses every 6 hours for 7 days.
- UTI prophylaxis: 1-2 mg/kg once daily or divided into two doses.
- Children over 12 years: 50-100 mg every 6 hours for 7 days or 3 days after obtaining a sterile urine culture.
- SR preparation: 100 mg twice daily for 7 days or 3 days after obtaining a sterile urine culture.
- Long-term prophylaxis/suppression: 50-100 mg at bedtime for up to 12 months.
Renal Impairment: Ofuran SR is ineffective in patients with creatinine clearance (CrCl) <60 mL/min due to inadequate urine concentration. Regular renal function monitoring is required, especially in elderly patients.
Side Effects
Common side effects include nausea, vomiting, diarrhoea, loss of appetite, dizziness, and headaches. Some patients may experience harmless brownish discolouration in their urine. Rare but serious effects include peripheral neuropathy, hepatotoxicity, anaphylaxis, interstitial pneumonitis, and pulmonary fibrosis. If severe reactions occur, immediate medical attention is required.
How to Use
Ofuran SR should be taken with food to improve absorption and reduce stomach upset. Swallow the tablet whole; do not chew, crush, or break it. For optimal results, take the medication at the same time every day. To prevent bacterial resistance, complete the entire course, even if symptoms improve early.
Pregnancy & Lactation
Ofuran SR is not recommended during the last stages of pregnancy (at term, labour, or delivery) due to potential harm to the newborn. It should be used cautiously during pregnancy only when the benefits outweigh the risks. Nitrofurantoin enters breast milk and may affect breastfeeding infants, so a doctor’s consultation is required before use.
Precautions and Warnings
Ofuran SR should be used cautiously in elderly patients and those with hepatic, renal, or pulmonary dysfunction. During prolonged therapy, regular liver and lung function monitoring is advised. Patients with G6PD deficiency, anaemia, electrolyte imbalances, or diabetes mellitus should use this medication with caution due to the increased risk of peripheral neuropathy. Discontinue use if any signs of neuropathy or severe allergic reactions appear.
Safety Information
Ofuran SR should not be used in the following:
- Severe renal impairment (CrCl <60 mL/min).
- Hypersensitivity to nitrofurans.
- Infants under 3 months old.
- Pregnancy at term, labour, or delivery.
Patients with a history of pulmonary or hepatic reactions to Nitrofurantoin should avoid this medication.
Indications
Ofuran SR is indicated for treating and preventing uncomplicated urinary tract infections, particularly those caused by E. coli and Staphylococcus saprophyticus.
Pharmacology
Nitrofurantoin inhibits bacterial enzyme systems, interfering with cell metabolism and cell wall synthesis. It has bactericidal activity against Gram-positive and Gram-negative urinary tract pathogens, ensuring effective UTI treatment and prevention.
Therapeutic Class
Urinary Tract Anti-Infectives (Nitrofurantoin Group).
Storage Conditions
Store at room temperature below 30°C, away from moisture and direct sunlight. Keep out of reach of children.
